HCDSB is now providing Medical Insurance coverage for our students through International Insurance.

Students who still have a valid StudyInsured insurance card should continue submitting claims through StudyInsured. Medical Insurance medical insurance is included with school tuition and is mandatory for all students.

If you are a parent, custodian or homestay family submitting the claim on behalf of a minor, please use the Minor Claim Submission Form.

If you are a student submitting the claim on behalf of yourself, please log into your “My Account” to complete the claim submission.

How to Access Medical Care in Canada

Student Immunization Record

Students attending school in our Board must provide their Immunization Record with proof of the vaccines they have received.

The most efficient way to provide your Immunization Record for school in Canada, is to use the Canada Immunization Record Form. Parents/Custodians are required to have this form completed and signed by a qualified health professional OR they may submit a copy of a student’s original Immunization Record from their home country to the Region of Halton (make sure the student’s name is clearly stated on the Immunization Record, and include the school they will be attending).  Please also complete the International Student Immunization Form and follow the instructions for submitting both forms by email to

The Halton Catholic DSB’s ISP Department staff will no longer collect and submit Immunization Records to the Halton Region Public Health Department.

PLEASE BE ADVISED: Failure to do this may result in a student being suspended from school.

Direct-Pay Medical Facilities

You won’t need to pay up front for your medical care for eligible expenses if you have an HCDSB Medical Insurance plan through

Simply present your Healthcare Access Card upon arrival at the Direct-Pay Medical Facility and payment will be taken care of for you.

Important Information

  • If you are sick or injured outside of the Halton Region, please use our “Find a Canadian Clinic Search” to locate the nearest Direct-Billing Medical Facility to your current location. 
  • If you are being admitted to the hospital or have been referred for expensive diagnostic testing (such as an MRI or CT scan), or need surgery, you are required to call’s Emergency Assistance number, 1-888-756-8428 (Toll-Free in North America) or 905-752-6230. This phone number is also found on the back of your Healthcare Access Card
  • If you do not call the Emergency Assistance number, or visit a clinic not on our preferred list, you may be asked to pay up-front. To be reimbursed, you will need to submit a claim
