How to Access Medical Care in Canada

Student Immunization Record

Students attending school in our Board must provide their Immunization Record with proof of the vaccines they have received.

The most efficient way to provide your Immunization Record for school in Canada, is to use the Canada Immunization Record Form. Parents/Custodians are required to have this form completed and signed by a qualified health professional OR they may submit a copy of a student’s original Immunization Record from their home country to the Region of Halton (make sure the student’s name is clearly stated on the Immunization Record, and include the school they will be attending).  Please also complete the International Student Immunization Form and follow the instructions for submitting both forms by email to accesshalton@halton.ca.

The Halton Catholic DSB’s ISP Department staff will no longer collect and submit Immunization Records to the Halton Region Public Health Department.

PLEASE BE ADVISED: Failure to do this may result in a student being suspended from school.
