guard.me Medical Insurance

guard.me Medical Insurance is included with school tuition, and is mandatory for all students.

guard.me pays the costs of medical services if you are unexpectedly sick or injured.

Submit a Claim

If you are a parent, custodian or homestay family submitting the claim on behalf of a minor, please use the Minor Claim Submission Form.

If you are a student submitting the claim on behalf of yourself, please log into your guard.me “My Account” to complete the claim submission. 

Direct-Pay Medical Facilities in Halton

You won’t need to pay up front for your medical care for eligible expenses when you have an HCDSB Medical Insurance plan through guard.me.

Simply present your guard.me Healthcare Access Card upon arrival at the Direct-Pay Medical Facility and payment will be taken care of for you.

Student Immunization Record

Students attending school in our Board must provide their Immunization Record with proof of the vaccines they have received to Halton Region Public Health.

The most efficient way to provide your Immunization Record is to use the Canada Immunization Record Form. Parents/Custodians are required to have this form completed and signed by a qualified health professional OR they may submit a copy of the student’s original Immunization Record from their home country to the Region of Halton (make sure the student’s name is clearly stated on the form, and include the school they will be attending).

Please also complete the International Student Immunization Form and follow the instructions for submitting both forms by email (email address is provided on the form).

The Halton Catholic DSB’s ISP Department staff will no longer collect and submit Immunization Records to the Halton Region Public Health Department.

PLEASE BE ADVISED: Failure to do this may result in a student being suspended from school.
